A reminder to Malaysian doctors that in the Dobbs forum, in one of the threads discussing ID notification, one Dobber has come up with a neat fillable PDF version. You just open up, fill in the fields and print. This thread is accessible to MMR doctors (Dobbs) forum members.

The notification form can be faxed to your nearest district health office – Searchable site from MOH
